What do Members need to know before submitting a claim for the Simple Critical Illness Program?
Members can file a claim three (3) months after joining the Simple Critical Illness ProgramMembers can make up to three claims for three different conditionsEach condition is considered a singular ailment for a claimA new claim needs to be filed for each separate conditionMembers need to provide necessary documentation as required by the Program, including, but not limited to, diagnosis and test results, prepared by the Member's doctor
